Findings: Alleged Victim (AV) tripped on the edge of a floor grate and fell, sustaining a skin tear and hematoma to head and arm. The facility failed to maintain a safe and hazard-free environment by having a floor grate that was raised above the floor, creating a tripping hazard, which is violation of resident’s rights, is considered neglect of care and constitutes abuse.
