Findings
The Alleged Victim (AV) is known to have aggressive behaviors and had previously been aggressive to Witness #3 (W3). Both were care planned to be kept separate from each other. On or about September 23. 2021, AV and W3 engaged in an unwitnessed altercation in W3's room where AV received minor redness to his/her hand. The facility failed to follow the care plan to ensure the safety the residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
