Findings
On or about September 26, 2021, Alleged Victim (AV) left the facility without assistance. An investigation determined that the back door of the facility was unlocked and AV left through this door. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
