Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about September 27, 2021, Alleged Perpetrator 2 (AP2) entered AV’s room and found furniture items obstructing AV’s doorway. AP2 failed to remove the obstruction from AV’s doorway, allowing ingress/egress from the room. AP2 failed to provide a safe environment to AV, which constitutes abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
