Findings
The Alleged Victim (AV) and Witness #1 IW1) were care planned for altercations and physical aggression. On or about September 26, 2021, AV and W1 were involved in an altercation where AV suffered a bruise to his/her face. The faciilty failed to follow the care plan to ensure the safety of the resident, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
