Findings
Alleged Victim (AV) Relies on the facility for his/her care. AV has a Foley catheter and is care planed for catheter care/maintenance. AV has history of recurring Urinary tract infections related to neglect of catheter care. Home Health provides catheter care and monitoring as needed. On or about September 15, 2021 Home Health reports fecal matter on tubing. The facility failed to follow the care plan which is a violation of resident right is neglect of care and constitutes abuse.
