Findings
Alleged Victim (AV) was care planned for showers twice a week, dining assistance and housekeeping. From approximately July 9, 2021, to October 15, 2021, AV’s care plan was not followed on multiple occasions regarding showers, dining assistance and housekeeping resulting in undesirable weight loss, unreasonable discomfort and a loss of personal dignity. The facility failed to follow the care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
