Alleged Perpetrator 2 failed to provide a safe environment when s/he left the facility residents without supervision for approximately an hour and half, placing residents at risk of serious harm and causing unreasonable discomfort to the Alleged Victim, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect the resident from neglect, which is a violation of Oregon Administrative Rules.
