Findings
On or about October 2, 2021, the Alleged Victim (AV) was upset because Witness #3 (W3) touched AV's thigh, causing AV to cry and be afraid of W3. W3 had been having sexualized behaviors recently. The facility failed to ensure the safety of opposite gender residents by not having interventions in place to ensure W3 was kept away from other residents. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
