Findings
The Alleged Victim (AV) had a history of wandering and had previously eloped. On or about October 03, 2021, he/she eloped from the facility campus, fell, and sustained an injury to his/her eyebrow. The facility failed to appropriately care plan and failed to provide a safe environment for AV. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
