Findings
Alleged Victim (AV) relies on the facility for h/h care. On multiple occasions AV was found soaked in urine or covered in feces after Alleged Perpetrator 2 (AP2) stated they provided care, causing AV unreasonable discomfort and/or serious loss of personal dignity. AP2 and the facility failed to ensure services were provided which is a violation of resident rights, is neglect of care and constitutes abuse.
