Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about October 11, 2021, AV received a text from the fraud department of his/her bank asking if he/she had made a purchase of jewelry in the amount of $98.54 using his/her debit card. AV replied no and was advised that his/her bank card would be canceled. AV last remembered using his/her debit card at a nearby convenience store on October 9, 2021 and indicated he/she is very careful about being sure to put it back in the case where he/she keeps it. Alleged Perpetrator 2’s (AP2’s), an unknown subject, actions are considered financial exploitation, which is a neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
