Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned for total assistance with toileting and/or continence care, AV requires one (1) person assistance with toileting and/or continence care. On multiple occasions after Alleged Perpetrator 2 (AP2) shift AV was found soaked in urine, including his/her brief and bedding causing unreasonable discomfort. AP2 and the facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
