On or about October 9, 2021, Alleged Victim (AV) was found in his/her bed without regular clothes on, without his/her Oxygen, and with an overflowing brief. The facility's failure to follow AV's care plan is violation of resident's rights, is considered neglect of care and constitutes abuse.
