Findings
According to documentation Alleged Perpetrator (AP2) and (AP3) failed to follow the facility protocols of a safe medication administration system. The failure resulted in the Alleged Victim (AV) receiving the wrong medications and needed to be placed on alert charting and experienced drowsiness and unreasonable discomfort, which is a violation of resident rights, is considered abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
