Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had several falls between August 4, 2021, and September 19, 2021. The facility failed to appropriately care plan for AV's fall risk and on or about, September 22, 2021, AV had another fall that resulted in a bruise on AV's head. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
