Findings
The Alleged Victim (AV) had approximately four (4) falls in four (4) months, from which AV was diagnosed with abrasions, bruising and pain to knees, cuts, scrapes and a sprain to right wrist. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
