Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had ten falls between August 7, 2021, to October 15, 2021, some with injuries. The facility failed to properly care plan to mitigate the risk of falls, and on or about October 18, 2021, AV had two additional falls with one resulting in skin tears. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
