Findings: The facility failed to provide a safe environment and proper supervision according to the Alleged Victim’s (AV) care plan and wandering behavior. The failure resulted in AV wandering into Witness 1's (W1) room and engaging in a physical altercation, causing emotional discomfort and injury to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
