Findings
The Alleged Victim (AV) had numerous falls where interventions were either not put in place or not followed. A fall occurring on or about October 20, 2021, resulted in a fractured hip, requiring surgical intervention. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
