Findings
The facility failed to provide a safe environment for the Alleged Victim (AV). On or about October 14, 2021, AV had an unwitnessed fall, resulting in AV having an abrasion on h/her right eye, and experiencing unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
