Findings
The facility failed to properly care plan for the Alleged Victim (AV) regarding transfers and fall risks related to AV that are not updated in the service. An incident occurred on October 20th, 2021 where AV had an unwitnessed fall resulting in a broken clavicle. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
