Findings
The Alleged Victim (AV) had approximately six (6) falls between August 08, 2021, to October 27, 2021. On or about October 27, 2021, AV fell resulting in a fracture to AV’s wrist. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
