Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about October 19, 2021, AV was found on the floor with a 5cm skin tear on his/her arm after he/she rolled or fell out of bed. AV had previously rolled/fell out of bed on October 15th and 17th, 2021, which both resulted in injury. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
