Findings
The facility failed to follow Alleged Victim’s (AV) care plan to complete safety checks every 30 minutes when AV is in his/her room, due to fall risk. On or about October 28, 2021, AV had an unwitnessed fall, resulting in elbow pain. The failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
