Findings
Alleged Victim (AV) relies on the facility to provide a safe environment. On or about October 29, 2021, AV knocked on the kitchen door and the door fell off the hinges striking AV. AV experienced headaches and a shoulder pain. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
