Findings
Witness 1 (W1) has a known history of resident-to-resident altercations. On or about November 2, 2021, AV was trying to open all doors looking for his/her bedroom. AV knocked on W1’s door and when W1 opened the door he/she shoved AV across the hall causing AV to fall and sustaining an injury. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
