Findings
Witness 1 (W1) is known to have aggressive behaviors. On or about October 31, 2021, W1 was showing aggressive behaviors. W1 kicked Alleged Victim and then punched a staff member. The facility failed to care plan related to W1’s known behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
