Findings
The facility failed to follow Alleged Victim’s (AV) care plan to lock AV’S bed breaks. The failure resulted in AV falling and experiencing pain on h/her left knee and back. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
