Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had thirteen falls between August 11, 2021, and November 7, 2021. The facility failed to properly care plan for AV's fall risk and on or about, November 9, 2021, AV had another fall where AV was sent to the hospital for a head injury that required staples. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
