Findings
The facility failed to provide basic care and safety for Alleged Victim (AV) by not providing adequate wound care and proper hygiene care which resulted in AV's wounds getting worse causing unreasonable discomfort due to AV being left in soiled briefs and dirty clothing. The facility's failure to properly care plan for AV's care needs is a violation of resident rights, is considered neglect of care and constitutes abuse.
