Findings
On or about November 15, 2021, the facility failed to follow the Alleged Victim's (AV) and Witness #1's (W1) care plan to keep the two separated from each other. AV and W1 engaged in an altercation where AV and W1 were choking and hitting each other, There were no injuries, however, AV and W1 were placed at risk of serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
