Findings
On or about November 12, 2021, Alleged Victim (AV) suffered a fall when Alleged Perpetrator 2 (AP2) pulled a chair out from under AV causing AV to fall to the ground and endure physical harm and unreasonable discomfort. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
