Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had fourteen falls between August 11, 2021, and November 9, 2021. The facility failed to properly care plan for AV's fall risk and on or about, November 14, 2021, AV had another fall where AV experienced pain. This failure is a violation of resident rights, is considered neglect of care which constitutes abuse.
