Findings
On or about November 17, 2021, it was determined that the facility failed to provide adequate services and care for Alleged Victim’s (AV) catheter treatment and facility also failed to maintain cleanliness and safety as directed in AV’s care plan. The facility failure to follow AV's care plan, which is a violation of resident's rights, is considered neglect of care and constitutes abuse.
