Findings
The Alleged Victim (AV) has a known history of falls. Between October 5, 2022 through November 6, 2021, AV sustained approximately seven (7) falls, all resulting in injury. On or about November 6, 2021, AV suffered a fall, sustaining a bump and bruise to the back of his/her head. AV was taken to the emergency room to be assessed and returned to the facility. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
