Findings
The facility failed to assess and intervene accordingly when the Alleged Victim's (AV) experienced a significant change of condition. On or about November 21, 2021, AV was transported to the hospital, in poor condition, and it was reported s/he had not been able to get out of bed for about a week. The failures are a violation of resident rights, is considered neglect of care and constitute abuse.
