The facility failed to appropriately care plan regarding alleged victims (AV) falls to ensure safety. AV fell approximately nine times between August 27th, 2021 and November 18th, 2021 with no new interventions put in place. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
