Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have assistance with toileting and transferring. AV has history of refusing assistance, toileting, and changing. AV was observed to be soiled, and bed/linens soiled. The facility failed to provide necessary interventions and care planning regarding AV refusing care, which is a violation of resident rights, is neglect of care and constitutes abuse.
