Findings
The facility failed to provide Alleged Victim (AV) with assistance with his/her mobility and hygiene resulting in AV getting two bilateral pressure ulcers on his/her buttocks and one pressure ulcer on his/her tailbone. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
