Findings
On or about November 25, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation that caused injury to AV when W1 grabbed AV's left arm causing a skin tear. W1's has a known history of resident-to-resident altercations. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
