Findings
Alleged Victim (AV) relies on the facility for his/her care. AV had eleven falls between June 1, 2021, and November 28, 2021. Based on interviews and facility documentation, eight of the eleven falls resulted in emergency medical treatment with the fall on November 28, 2021, causing AV a head injury. The facility failed to appropriately care plan and implement proper interventions to mitigate AV's fall risk. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
