Findings
The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to change of condition when AV experienced a physical decline. AV experienced unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is neglect of care which constitutes abuse.
