Findings
The Alleged Victim (AV) and Witness #1 (W1) were care planned for having challenging behaviors and engaging in altercations. On or about December 2, 2021, W1 became agitated and pushed AV out of the room, causing AV to fall backwards. AV was not injured, however, AV was placed at risk for harm. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
