- Alleged Victim (AV) and Witness 1 (W1) are known to have resident-to-resident altercations. W1 is care planned to be redirected whenever he/she enters the room of other residents. On or about December 6, 2021, AV and W1 entered the room of another resident in which the other resident became upset and started yelling at AV and W1 to get out. W1 responded to the matter by becoming upset and punching AV in the face and giving him/her a black eye. The facility failed to provide AV a safe environment which is a violation of resident rights, is considered neglect of care and constitutes abuse.
