Findings
The Alleged Victim (AV) has had multiple falls between June 1, 2021 and September 24, 2021. Falls were with and without injury, none of the falls were reported to Adult Protective Services. On or about August 4, 2021, AV was found with a bruise from unknown origin. There are no interventions in place to ensure AV's safety from falls. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
