Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about 12/07/2021, Alleged Perpetrator 2 (AP2) was assisting AV with his/her jacket zipper when the zipper separated and broke. AV said, “you broke it” and AP2 called AV a liar, which is considered verbal/emotional abuse. The facility failed to protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
