Findings
AV experienced approximately eight falls from September 5, 2021, to December 4, 2021. On or about, December 7, 2021, AV went to the hospital and was diagnosed with a blood clot. It was also discovered during the investigation, that AV's care plan was not being followed relating to medication, housekeeping and toileting due to a lack of staffing. The facility failed to provide appropriate services according to resident needs. These failures are a violation of resident rights, is neglect of care which constitutes abuse.
