Findings
On or about December 10, 2021, Alleged Victim (AV) was sent to the hospital for treatment due to AV suffering a fall with injury. Per EMS records, AV was found covered in his/her own urine and stool and AV's room at facility had strong urine and feces smell. AV was treated/discharged from hospital but was sent back to hospital again on December 11, 2021, for further treatment due to AV showing signs of having an altered mental state. Per EMS records, AV was found covered in urine and feces again, lying on the floor in the sheet that AV was discharged with from the hospital the day prior. An investigation determined that the facility failed to provide appropriate care to AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
