- The Alleged Victim (AV) was care planned to be assisted with toileting. On or about December 12, 2021, AV was found soiled in urine and the condition of his/her room was unsanitary. Due to a lack of appropriate staffing, AV’s care plan was not followed, leaving AV at risk for harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
